All OHTs are responsible to provide patients, families and caregivers with enhanced 24/7 supports regarding navigation to care and improved transitions between settings. The Couchiching OHT is always working on enhancing system navigation across health teams in Couchiching in to ensure that no door is the wrong door for patients seeking care.
The 24/7 System Navigation Project aims to develop a patient navigation system for patients and their caregivers to enable them to seamlessly and successfully navigate every aspect of their care needs. The design will be ever-evolving but initially includes a simple one-number approach to streamline how the community contacts various providers.
