News Release: Couchiching OHT Launches Comprehensive Chronic Disease Program & New Hub Location

The Couchiching Ontario Health Team (COHT) is thrilled to announce the launch of the COHT Chronic Disease Program, a significant step forward in providing comprehensive and integrated care for patients with chronic conditions. This initiative begins with a focus on Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD) within the local community.

The COHT Chronic Disease Program aims to create a highly integrated system of care that benefits patients, caregivers, and healthcare providers across Couchiching. By fostering strong collaborations among our numerous partners, the program ensures seamless service delivery and navigation within our community.

Key Goals of the COHT Chronic Disease Program:

  • Integration Across Care Partners: Uniting various care providers to offer a streamlined and cohesive approach to patient care with navigation support.
  • Early Intervention: Delivering integrated care earlier in the patient journey to reduce the necessity for acute care or emergency hospital interventions.
  • Co-Design: Developed in collaboration with patients, clinicians, and specialists, the program is tailored to meet the diverse needs of our community effectively.

Building on the success of the “Couchiching Family Health Team Heart Function Clinic,” these pathways are the result of extensive co-design efforts and are intended to provide comprehensive, patient-centered care. This program will be anchored through the newly established COHT Chronic Disease Hub, located at 119 Memorial, Orillia, operated by the Couchiching Family Health Team.
