Submissions are monitored between 8am to 4pm, Monday through Friday. Please allow two business days for a member of our team to get back to you. If you need services outside these hours, please refer to the services listed below for 24/7 support.

Due to the high demand for a primary care practitioner, we are currently only able to accept patients onto our waitlist who permanently live in the Couchiching area, including Ramara, Rama First Nation, Oro-Medonte, Severn, and Orillia, who do not have a family doctor or nurse practitioner in Couchiching.

    The Couchiching Family Physician & Nurse Practitioner Waitlist is a community initiative by our Ontario Health Team (OHT) to assist residents who do not have a family doctor or nurse practitioner.

  • Why are we creating our own Couchiching Family Physician & Nurse Practitioner Waitlist?

    We are creating our own list to better support unattached residents of our community connect with available healthcare providers more efficiently. By managing the list locally, we can better understand and address the specific needs of our community.

  • How do I add my name to the Couchiching Family Physician & Nurse Practitioner Waitlist?

    You can add your name to the list by filling out the simple online form above.

  • Who is eligible to join the Couchiching Family Physician & Nurse Practitioner Waitlist?

    Any permanent resident of Couchiching who does not currently have a family doctor or nurse practitioner is eligible to join the waitlist. Reminder that the Couchiching area covers Ramara, Rama First Nation, Oro-Medonte, Severn, and Orillia,

  • How will I be matched with a healthcare provider? 

    Once you are on the list, our team will work to match you with a healthcare provider based on availability, your health needs, and your location. We aim to make the process as seamless as possible.

  • How long will it take to be matched with a healthcare provider?

    The time it takes to be matched can vary depending on the availability of healthcare providers and the number of people on the list. We strive to match patients as quickly as possible and will keep you informed throughout the process.

  • What if I need medical care while waiting to be matched?

    If you need medical care, please visit the COHT Unattached Clinic. For emergency situations, please visit the Emergency Department.

  • Can I update my information after joining the waitlist? 

    Yes, you can update your information at any time by contacting Couchiching OHT. Keeping your information up-to-date helps us match you with a healthcare provider more effectively.

  • How will my personal information be protected? 

    All personal information collected for the Couchiching Family Physician & Nurse Practitioner Waitlist will be managed by the Couchiching Family Health Team, kept confidential and used solely for the purpose of matching patients with a healthcare provider.
