Become a PFCN Member
This collaboration and co-creation approach continues today through PFCN Chair membership at the COHT Joint Executive Team and PFCN participation in all working groups to inform and provide the patient, family, and caregiver perspective in all COHT deliverables.

The Couchiching Ontario Health Team (COHT) is seeking additional Affiliate Members to join our team. COHT Affiliate members play a vital role in how the COHT collaborates with system partners and subject matter experts in the field of health and social services across the Region.
COHT Affiliate members are:
- Are committed to the shared guiding principles and core values of the COHT
- Contribute to the Collaboration table (bi-monthly) meeting to provide insights, advice and support on COHT project and progress presentations
- Provide expert advise on areas specific to the organization they represent
- Provide input to decision making and participate in projects and initiatives where possible to support common goals of the COHT
- Overall, Affiliate members are committed to continuing the development and achievements of the COHT; passionate about the work towards integrated care systems and solutions for patients and care providers in Couchiching.
- Patient, Family & Caregiver Network Members – The COHT Patient, Family & Caregiver Network (PFCN) is seeking volunteers who would like to participate in the co-design and support of the COHT’s goals, projects and working groups. Become an Anchor Member
The Couchiching Ontario Health Team (COHT) is seeking additional Anchor Members to join our team. COHT Anchor members play a vital role in the design, implementation and oversight of integrated care growth in our Region.
COHT Anchor members are:
- Committed to the shared guiding principles and core values of the COHT
- Committed to working collaboratively with anchors, affiliates and all partners of the COHT in order to co-create a high-quality, integrated care system for Couchiching residents
- Signatory members on the Collaborative Decision Making Agreement (CDMA) – contract
- Involved in decision-making, planning and design for strategic initiatives and system design projects
- Contribute to the overall success for the COHT by leveraging financial and or in-kind resources to support the common goals, vision and mission of the COHT
Become a Primary Care Network Member
The COHT Primary Care Network is a collection of primary care providers, inclusive of family physicians, nurse practitioners, midwives, community pharmacists, indigenous health providers, and community paramedics who are working collectively to improve the patient and provider experience within the Couchiching region.
