Joanne Matte – Compassionate Care Champion

Congratulations to Joanne Matte, a Nurse Practitioner as well as Palliative Care with Ontario Health atHome
Joanne is nominated in the Compassionate Care Champion category for exceptional patient-centred care. Joanne was nominated anonymously– here is what her nominator said:
“Joanne is compassionate, smart, in tune with patient needs and broadly skilled in palliative care and general primary care. She works hard and she never fails to support the patients and families in front of her, as well as her team members. She is a trusted high-performing colleague who communicates in all the right ways with the physicians with whom she shares care. She is self-effacing, flexible and courageous. Her patients and their families are lucky to have her care for them. And her colleagues also benefit from her approach and caring and top-notch communication skills.
Joanne and I have shared care for palliative patients over the years, and Joanne does all the work and does it well. The families and patients are so well looked after, and the communications back to me are excellent and make it easy for me to provide overnight and weekend backup. Joanne provides high high-quality day-to-day in-home palliative care that I can’t due to other clinical commitments. She is a model of how efficient high-quality integrated team care can work. She is the reason I can offer to support additional palliative patients in the community and across a larger geography. We had a particularly difficult case not that long ago and I will be forever grateful to Joanne for her attentive care and her support to me as I was challenged and so saddened by the circumstances. I am so grateful to Joanne and want to say thank you and hoot hoot hooray!”
