MEMO – Update #3: Expanded Interdisciplinary Primary Care Teams in Couchiching

Following our previous updates, we are excited to share the successful expansion of our interdisciplinary primary care teams across the Couchiching region. This expansion, made possible through additional funding aimed at improving access to care, will greatly enhance our ability to provide comprehensive services to both attached and unattached patients in the community.

As part of this initiative, the Couchiching Family Health Team (CFHT) and the Huronia Nurse Practitioner-Led Clinic (HNPLC) have broadened their interdisciplinary primary care services to better meet the needs of our community. See further detail on access to care expansions below, and further communications will be circulated more broadly in the coming days. Please share with your respective partners.

• HNPLC Interdisciplinary Team Expansion:

1. The HNPLC has successfully recruited additional Nurse Practitioners to support:

• New Children’s Clinic, a dedicated Children’s Clinic at the County Orillia Campus (250 West Street N), which will focus on paediatric care for underserved children in Couchiching. This clinic is now accepting patients.
  • Hours of Operation: 8:30am – 4:00 PM

• Lakehead University Clinic partnership
• County of Simcoe, Social & Community Services Clinic
• Paneling 1,000+ patients to ensure attachment of those who are most medically complex and in need within our community

2. CFHT Interdisciplinary Team Expansion: The CFHT has expanded its team with new members to enhance support for:

• The COHT Unattached Clinic, designed to provide services to patients without a primary care provider. This clinic will help address the ongoing need for access to primary care in the region. This clinic is now accepting patients.
  • Hours of Operation: Monday – Thursday, 8:30am – 4:00pm, Friday 8:30pm – 3:00pm
  • Booking Phone Number: 705-329-3649 ext. 231 (online booking coming soon!)
  • Patients can self-refer or be referred by healthcare providers working in the field of navigation: Referral Form here 
  • Same day appointments can be available
• Additionally, all individuals in Couchiching can access the following services at the CFHT:
  • Trans Health Services (self-referral available)
  • Chronic Disease Program
  • CARE Exercise Program
  • Geriatric Outreach Team
  • Ontario Structured Psychotherapy (self-referral available)
  • Palliative Care (self-referral through 705-623-3176)
  • System Navigation Team (self-referral through 1-844-806-3093)
