News Release: Couchiching OHT Announces Expansion of Interdisciplinary Primary Care Teams

The Couchiching Ontario Health Team (OHT) was proud to announce this past February, that it had been officially acknowledged by the Ministry of Health (MOH) and Ontario Health (OH) for its outstanding proposal to expand interdisciplinary primary care team services to its community. This recognition highlights the OHT’s commitment to improving access to care and enabling access to the right care, in the right place for patients who otherwise do not have access to a healthcare provider.

As health system partners, the Couchiching OHT has played a crucial role in fostering collaboration, patient and caregiver co-design and integration within the healthcare system. By embracing an integrated co-design approach, the Couchiching OHT is dedicated to enhancing the overall health and well-being of the community it serves. As part of this primary care expansion initiative, the COHT, in collaboration with the Couchiching Family Health Team (CFHT) and the Huronia Nurse Practitioner-Led Clinic (HNPLC) operational teams, will be broadening their interdisciplinary primary care teams to better cater to the needs of our community. In the upcoming weeks and months, the CFHT will offer supplementary support for allied health services and programs, while also extending primary care services to both attached and unattached patients in the area. Simultaneously, the HNPLC will expand its services with a focus on paneling 1,000 patients, ensuring attachment for those who are most medically complex and in need within our community.

In light of these positive expansions, it is with mixed emotions that we announce the closure of the COHT-OSMH COVID Care Clinic. This clinic, born out of our COVID operations, has been instrumental in our community’s pandemic response and recovery efforts for nearly four years. We extend our heartfelt gratitude to the Care Clinic team and all staff who have worked tirelessly in collaboration with them during this pivotal time in our community’s history. The COHT-OSMH COVID Care Clinic will cease operations effective June 21, 2024, with the team actively ensuring continuity of care for current patients during the winding-down process.

In the coming weeks, the CFHT and HNPLC will ramp up their operations, beginning in June. The COHT will disseminate information regarding access to care initiatives through its social media channels and newsletters.
