Patients and Families have worked collaboratively with the COHT from the initial application submission in 2018. This collaboration and co-creation approach continues today through PFCN Chair membership at the COHT Joint Executive Team and PFCN participation in all working groups to inform and provide the patient, family, and caregiver perspective in all COHT deliverables.
The Couchiching Patient, Family, Caregiver Network works together to co-create the work of the Couchiching OHT to ensure a voice of the patient, family and caregiver. Our PFCN is involved in project planning, strategic work and all efforts carried forward by our Team. Our PFCN is integral to the growth and design of the work our Team is building to ensure seamless and integrated care for our Community.
