Congratulations to the PSWs and Case Manager of the LOFT Hub and Spoke Team.

The Hub and Spoke Team is nominated in the Compassionate Care Champion category for exceptional patient-centered care. The team was nominated by Stephanie Boehm, Coordinator of Complex Care and Assisted Living for LOFT’s Hub and Spoke program. Here is what Stephanie had to say:

“This team I have the luxury of overseeing never quits! During ice storms they boil water at home and go to client’s with a thermos and a cup of soup package. When no one else will enter, the LOFT Hub and Spoke team is always there. Clients and client families all acknowledge the amazing work that this team does. We have clients who have shared stories of how they would be dead, if not for LOFT’s H&S Team. They work together and with external agencies to ensure that all care is being provided. When it looks impossible, they find a way! They text, call, and drop by the office to support one another in their support journeys. When there is a client need that can’t be addressed by one, the team doesn’t need to be asked. They are competing over who is going to get there for assistance first! I can never say enough about what a joy and honor it has been to oversee such an amazing crew of dedicated and unwavering workers.

There are too many stories. I’ll share this. A client discovered they had an infestation. Our team was able to connect them with a pest service provider and schedule the appointment for less than a couple of days away. The team worked feverishly to prepare the home and navigate client relocation for her and her two small dogs. This may not sound like a big task, but with the circumstances that this client faced, it was monumental! The team worked to support the client out of their home and in the community for many hours and then return her to her home. Her and her family were very appreciative of all the support that was provided and were thrilled with the complete attention to detail. The team worked to have all the home cleaned and bagged. They connected with other external supports and provided timely updates and direction. There were a thousand little pieces to this situation. They did it all! They didn’t need to be asked. They didn’t think twice. They just did what needed to be done, to provide the best person-centered care imaginable. I wish I could tell you all the ways that this team impacts everyone. But if I started writing now, I would never finish.”
