As noted in our previous update, we are pleased to announce the successful expansion of our interdisciplinary primary care teams within the Couchiching region, thanks to additional funding awarded to enhance access to care in our community. This development will significantly improve our ability to provide interdisciplinary care to both attached and unattached patients in the region.
As part of this initiative, the Couchiching Family Health Team (CFHT) and the Huronia Nurse Practitioner Led Clinic (HNPLC), will be expanding their interdisciplinary primary care team services to enhance access to care for our Community. Updates on the success of these enhancements and expansions are as follows:

The CFHT is actively recruiting for the new Unattached Care Clinic, set to open this summer (referral form and memo will be announced). In the interim, patients without a primary care provider should note they can access the following services at the CFHT:

With these positive expansions, we remind you that the COHT Covid Care Clinic has closed their doors as of June 21st. This clinic, established during our COVID operations, has been vital to our community’s pandemic response and recovery for nearly four years. We sincerely thank the Care Clinic Team their tireless efforts and collaboration during this important period in our community’s history.
