What do you need to complete the survey?
– Access to the internet and a device (such as a computer, smartphone, or tablet)
– Five to ten minutes of your time
Why does your voice matter?
By sharing your thoughts, you will help health care providers, and the health care system learn:
– If you felt respected and engaged in your care
– How well you feel your health care providers communicate and work with each other
– How prepared and supported you feel to manage your health after your appointment(s)
– Who you are as a patient, such as your race and gender (if you feel comfortable sharing)
How can you take this survey?
You can complete the survey at any time by using:

Who can complete the survey?
Any patient who got care for their physical or mental health within the last three months. You can also complete this survey on behalf
of a patient, with their consent, if you are:
– A parent or guardian of the patient
– A caregiver for the patient
– Another family member of the patient
– A support person for the patient
