What is a Primary Care Network?

The Primary Care Network (PCN) aims to connect, integrate, and support primary care providers. Within the Couchiching region, we aim to bring together providers and create a sense of community to collectively improve health outcomes and provider experience.

What are the Benefits?

  • Up to date local referral pathways and services
  • Leadership opportunities and education funding
  • Mentorship and peer support

Why is a PCN Needed?

The aim of a Primary Care Network is to enhance the capacity of primary care providers to support each other and their patients – now and in future.

How does Couchiching support Primary Care Networks?

The Couchiching Ontario Health Team is proud to have dedicated clinicians in key roles to drive projects forward and ensure informed decision-making at the executive level. These clinicians bring their expertise to various initiatives, enhancing the quality and coordination of care. By actively participating in executive discussions, they provide valuable insights that shape the strategic direction of the OHT, ensuring that patient care remains at the forefront of all decisions.

Overall, Primary Care Networks are essential for creating a connected healthcare system in Ontario. By organizing and supporting primary care providers, PCNs help improve patient and provider experiences and health outcomes, making care more accessible and equitable for all.
